What You’ll Do 

RNs: 

• Conduct skilled nursing visits across a range of needs (may include Start of Care, Resumption of Care, Recertifications, Wound Care & Pediatric Visits). 
• Collaborate with care teams to ensure quality outcomes 
• Complete timely documentation via tablet 

Therapists (PT, OT, SLP): 

• Deliver home-based therapy services according to the plan of care 
• Conduct evaluations and develop goal-driven therapy plans 
• Support patient independence through customized therapeutic interventions 
• Coordinate care and progress with an interdisciplinary team 

What You’ll Need:

• Active New York license (RN, PT, OT, or SLP) 
• Reliable transportation and a valid driver’s license 
• OASIS knowledge preferred for RNs (not required) 
• Home care or rehab/acute care experience preferred 
• Familiarity with Homecare Homebase (HCHB) is a plus
